How do I add multiple choice questions to my form?

Ask your audience to choose from a range of options, add images to the options, allow them to select more than one option, and change the field appearance! To add a Multiple Choice field to your form, follow our step-by-step instructions below:

Using slash commands

  1. On a blank line in the form editor, type /multiple and select "Multiple Choice" from the popup menu.

  2. Enter details for the question title and description.

  3. Select the question to open the right-side Configuration panel to set up question-specific behavior, such as choosing more than one answer and Conditional Logic.

Using action buttons

  1. In the form editor, click the line where you want to add a multiple choice question. Click the quick action plus icon then "Add questions", or click the Add another question button below a previous question.

    Select a blank line, and click the plus sign and Add question from the popup menu on the left, or click the Add Question icon below a previous question.
  2. Select "Multiple Choice" from the question type dropdown on the right of the new question.
